Abstract [sv]

Inledning: Av cirka 44 000 intensivvårdstillfällen i Sverige under år 2015 avled 7,5 % av patienterna på en intensivvårdsavdelning. Palliativ omvårdnad är en del av intensivvården då behandlade vård kan avslutas och övergå i ett palliativt skede. Denna övergång skapar en transitionsprocess hos intensivvårdssjuksköterskorna vilket innebär omställning och anpassning av sin roll till den nya situationen. Syfte: Syftet var att beskriva intensivvårdssjuksköterskors upplevelser av att övergå från behandlande till palliativ omvårdnad. Design & Metod: Totalt 12 stycken intervjuer analyserades med kvalitativ innehållsanalys med deduktiv ansats av transitionsteorin. Resultat: Huvudfynden var att beslutet om att inleda palliativ vård inte väckte negativa känslor men att tiden innan var frustrerande då beslutet försenades och gav patienten förlängt lidande. Att skicka döende patienter till en vårdavdelning var frustrerande då en ny transitionsprocess påbörjades och inte gav intensivvårdssjuksköterskorna ett avslut. Konklusion: Intensivvårdssjuksköterskornas transitionsprocess skapar starka känslor vilka behöver bearbetas. Genom utbildning skapas förståelse för det multiprofessionella teamets olika roller och vad de kan bidra med och detta kan tydliggöra och ge intensivvårdssjuksköterskorna en formell roll i diskussionen inför beslutet.

Abstract [en]

Introduction: In Sweden during 2015 there were nearly 44 000 admissions in the intensive care unit (ICU) and in total 7. 5 % of the patients died during their stay in the ICU. Palliative care belongs in the ICU when treatment is ended. The change in treatment initiates a transitionprocess in nurses that triggers a readjustment and adaption to the new situation. Aim: The aim was to describe intensive care unit nurses experiences of transitioning to palliative care. Design & Method: In total were 12 interviews analyzed with qualitative content analysis with a deductive approach of the transition theory. Findings: The main findings were that the decision did not awake any negative feelings but a delayed palliative care decision was frustrating because the patient was suffering. Transferring dying patients to the ward was frustrating because it initiated a new transition phase and the closure the ICU nurses needed was missed out. Conclusion: The ICU nurse transition process engenders feelings which need closure. The ICU nurses role in the end-of-life discussion should be clarified and given formality through education and understanding of the multi-professional team.
