Abstract [sv]

Bakgrund: Svenska regeringen har beslutat om rikssjukvård, vars syfte är att höja vårdens kvalitet och använda dess resurser mer effektivt. Detta innebär att patienter remitteras mellan regionsjukhus för att få högspecialiserad hälso- och sjukvård. En del av dessa patienter kräver ambulanstransport och i vissa fall extrakompetens i form av anestesisjuksköterska vid svårt sjuka patienter. Ambulanstransport mellan sjukhus benämns interhospitala transporter.

Syfte: Syftet var att beskriva anestesisjuksköterskans upplevelser av interhospitala transporter.

Metod: Semistrukturerade intervjuer genomfördes med 17 anestesisjuksköterskor på fyra sjukhus i södra Sverige. En kvalitativ innehållsanalys genomfördes enligt Hsieh och Shannon (2005).

Resultat: Interhospitala transporter upplevdes av informanterna vara en av den mest krävande arbetsuppgiften i deras yrkesutövning. Ur analysen framkom tre kategorier av anestesisjuksköterskornas upplevelser: upplevelser kring kontroll, upplevelser kring trygghet och upplevelser kring utsatthet.

Konklusion: Anestesisjuksköterskornas upplevelse kring kontroll berodde på mental förberedelse, tydliga ordinationer samt stöd från läkaren. Det som ökade upplevelsen kring trygghet under transporter beskrevs i studien vara kännedom i teamet, extra narkoskompetens och yrkeserfarenhet. Anestesisjuksköterskorna upplevde också utsatthet och att har för stort ansvar. Detta upplevdes framförallt vid att behöva intubera i en utsatt miljö, ta svåra beslut ensam och att transportera svårt sjuka patienter.

Abstract [en]

Background: The Swedish government has decided on a nationwide policy on hospital care. The aim is to take better care of available resources, raise quality and work more efficient. As a part of this way of work patients are referred to, and between, region hospitals in order to get specialized health and medical care. In some cases this also involves transportations by ambulance between hospitals with support from a anesthetic nurse. Transports by ambulance between hospitals is called “interhospital transports”.

Aim: The aim of this paper is to describe the experiences of the anesthetic nurse during interhospital transports.

Method: Semistructured interviews were performed with 17 anesthetic nurses at four hospitals in the southern region of Sweden. A qualitative content analysis were performed according to Hsieh and Shannon (2005).

Result: Interhospital transports are experienced as one of the most demanding tasks in these

nurses work. In this analysis three main categories of experiences were found. These three

categories were: Control, comfort and the feeling of being left out with great responsibility.

Conclusion: The experience of control, at the participants of this study, was when they had

time to prepare mentally, and had a clear prescription from the responsible doctor. The feeling

of comfort, by the participants, during transports was strongly connected to the level of

knowledge in the team, in for example anesthetics but also medical work experience in

general. The participants also experienced great vulnerability when being left with too much

responsibility. This vulnerability was mostly felt by the feeling of having to be ready to

endotracheal

intubation

in an unknown location, far from hospitals, and to make difficult

decisions alone, all this with severe sick patients.
