    Abstract [en]

    Continuous positive airway pressure (CPAP) is the treatment of choice for obstructive sleep apnoea syndrome (OSAS), but side effects are common and long-term adherence low. The Type D (distressed) personality is defined as a combination of negative affectivity and social inhibition. The association of Type D personality with adherence has not been studied in CPAP-treated patients with OSAS. This study aimed to describe the prevalence of Type D personality in OSAS patients with CPAP treatment longer than 6 months and the association with self-reported side effects and adherence. A cross-sectional descriptive design was used. A total of 247 OSAS patients with a mean use of CPAP treatment for 55 months (6-182 months) were included. Data collection was achieved by two questionnaires, the Type D scale 14 (DS14) (Type D personality), SECI (side effects of CPAP), as well as from medical records (clinical variables and objective adherence to CPAP treatment). Type D personality occurred in 30% of the patients with OSAS and significantly (P < 0.05-0.001) increased the perceived frequency and severity of a broad range of side effects. The objective adherence was significantly lower (P < 0.001) for OSAS patients with Type D compared to OSAS patients without Type D, both with regard to a mean use of 4 h per night and 85% of the self-rated sleep time per night. The additional effect of a Type D personality on perceived side effects and adherence to CPAP treatment found in this study could be used by healthcare personnel when evaluating patients waiting for treatment. © 2007 European Sleep Research Society.

    Aim: Research focusing on hyperarousability in association with general sensitivity to stress has increased. This study aimed to: (i) describe values for self-reported hyperarousal behaviour traits, depression, sleeplessness behaviour and health-related quality of life [The Short Form 36 Health Survey Questionnaire (SF-36)] in a gender-stratified random sample from the Swedish population, and (ii) test the validity and reliability of the Swedish version of the Hyperarousal Behavioural Trait Scale (H-scale). Methods: In this study, 402 women and 391 men from Sweden were included. A test-retest study was performed on 297 subjects. Results: The total mean score on the H-scale was 29.5 (SD 10.0, 95% CI 28.8-30.2). Compared to men, women scored higher on the H-scale (total score, sub-scales and many items), whereas no evidence of an age trend was seen. The H-scale has proven to be a valid and reliable scale. Pearson's correlation coefficient showed similar magnitude and direction between the H-scale and the Zung's Self-rating Depression Scale, as between the H-scale and the Vicious Cycle of Sleeplessness Behaviour Scale, Vitality, Mental Health and the Mental Component Summary index on the SF-36 respectively. The Cronbach's alpha for the H-scale was 0.84 and estimated stability test-retest point of time varies between 0.73 and 0.80. Conclusions: This study indicates gender differences in response style in association with altered health-related quality of life. The H-scale is a valid and reliable self-reported scale for measuring hyperarousal behavioural trait research outcome in clinical practice. © 2006 Nordic College of Caring Science.

    OBJECTIVE: The primary aim of this study was to systematically compare perceived sleep quality, sleeplessness behavior, sense of mastery, self-esteem, depression, subjective health, and effects of sleep loss in men and women with stable coronary artery disease (CAD). Further aims were to determine possible predictors of poor sleep quality and sense of mastery, as well as the consequences of too little sleep. METHODS: Comparative-correlation and predictive design were used. Patients with a history of stable angina pectoris scheduled to undergo coronary angiography at Linköping University Hospital in Sweden were included. There were 47 women and 88 men (mean age 62.4 years) with CAD. Structured interviews using validated questionnaires covered sleep quality and sleep habits, effects of sleep loss, psychologic resources, and depression. RESULTS: Multiple stepwise regression analysis showed that sleeplessness behavior, depressed mood, female gender, and pharmacologic treatments with inflammation inhibitors significantly (P < .0001) accounted for the variance of poorer sleep quality. The analysis also showed that the following factors in descending order significantly accounted (P < .0001) for the outcome of sleep quality: inability to feel refreshed by sleep, difficulty in maintaining sleep, gastrointestinal problems, too little sleep, final morning awakening time, sleep onset latency, lying down because of daytime tiredness, and daytime physical tiredness. CONCLUSIONS: Compared with men, women with stable CAD may be especially at risk of experiencing poor sleep quality, even when sleeplessness behavior and pharmacologic treatments with inflammation inhibitors are controlled.     Abstract [en]

    The present work consists of clinical and experimental studies and describes some effects of metabolic disturbance on nervous function. It is an electrophysiological examination of both single cells and patients exposed to metabolic insult or disease. The patch-clamp technique has been used for measurements of whole-cell currents in rat brain slices and studies of nerve conduction has been used as a clinical test on peripheral nervous function in young patients with insulin-dependent diabetes mellitus (IDDM).

    The objective of the clinical studies was to find out to what extent young IDDM patients with multiple insulin injection therapy (MIT) still develop peripheral nerve dysfunction and to elucidate the reason why.

    The objective of the experimental studies was to evaluate the effect of metabolic disturbance - induced chemically or by anoxia - on the membrane potential and the ionic conductances in hippocampal CA1 pyramidal cells.

    It is concluded that

    (1) despite MIT-treatment and good metabolic control nerve dysfunction is still common in an unselected group of children and adolescents with IDDM

    (2) the most important risk factors for nerve dysfunction in IDDM arc increased height and poor long-term metabolic control

    (3) metabolic inhibition at 22-24°C or anoxia at 33-34°C causes hyperpolarization and changes in the function of several types of K+ channels in CA1 pyramidal cells

    (4) a change in membrane potential by a few millivolts inhibited the spontaneous impulse firing

    (5) a transient opening of tolbutamide-sensitive K+ channels could explain the increase in grest and the hyperpolarization observed in most cells during metabolic inhibition

    (6) by this mechanism the excitablity decreases which may diminish the energy demand in cortical cells during different types of metabolic insult.

    This follow-up study examines whether the development of nerve dysfunction is retarded by tight metabolic control in patients with type I diabetes mellitus. Seventy-one patients and 115 age-matched healthy control subjects underwent studies of nerve conduction in peroneal and sural nerves. The presence of diabetes was associated with a reduction in peroneal motor nerve conduction velocity (MCV) by 5.9 m/s, sural sensory nerve conduction velocity (SCV) by 3.4 m/s, and sural sensory nerve action potential (SNAP) amplitude by 22%. Dysfunction in peroneal MCV, sural SCV, and sural SNAP were related to long-term poor metabolic control. Eleven of 12 patients with HbA1c <6.5% had normal nerve conduction or abnormality in only one nerve as compared to 2 of 15 patients with HbA1c >8.0%. It is concluded that tight long-term metabolic control (HbA1c <6.5%) can retard nerve dysfunction in patients with type I diabetes mellitus and a mean disease duration of 12 years.

    Abstract [en]

    Depression is common among patients with chronic heart failure (HF) and leads to more symptoms of HF, decreased quality of life, and an increased risk for premature death. Depressed HF patients also use more health care resources, which increases the economic burden on the health care system. The assessment of risk factors of depression such as age younger than 60-65 years, poor physical functioning, previous depression, poor self-efficacy, living alone, and distressful relationships, in combination with the use of depression instruments, can be helpful in detecting depression in HF patients. Unfortunately, interventions on how to relieve depression in patients with HF have not been investigated thoroughly, however, depression agents as well as HF education, social support, exercise therapy, stress management, and relaxation have been shown to be useful interventions. Because of poor outcomes, studies that examine the effectiveness and/or side effects of pharmacologic as well as nonpharmacologic interventions on depressed patients with HF are needed.

    Ion Transport in Glial and Neuroblastoma Cells1997Doctoral thesis, comprehensive summary (Other academic)
    Abstract [en]

    The aim of this thesis has been to increase our understanding of the ion channel function and the ion transport mechanisms in glial cells. The analysis has also covered Na channel function in human neuroblastoma cells in order to get a basis for similar studies of glial cells In paper I isotopes 42K and 201 Tl were used to compare the K+ transport properties in cell lines from human malignant gliomas (U-251MG and Tp-378MG), carcinomas, and brain metastasis. In papers II and Ill the patch-clamp technique has been used in studies of the membrane potential and K+ conductance in human glioma cells (U-251MG) and in astrocytes of primary cultures from newborn rats. Paper IV is a patch clamp analysis of the Na+ channel voltage dependence in a human neuroblastoma (SK-N-SH) and esthcsioneuroblastoma (Tp-410) cell line.

    From these studies it was concluded that:

    (1) Cultured human glioma cells have a high specific K+ and Tl+ uptake of which approximately 90% is carried by Na,K-ATPase-dependent transport and Na-K-Cl cotransport. Inwardly rectifying K+ channels seem to have little importance for the K+ -uptake, at least in isolated cells.

    (2) The membrane potential in rat astrocytes hyperpolarizes while human glioma cells depolarize in low concentrations of K+. This difference may be related to the effect of K+ on inwardly rectifying K+ channels which were present in higher density in human glioma cells.

    (3) ca2+ free solutions depolarize the cells which seem to involve a transformation of K+ channels to unspecific leakage channels in rat astrocytes. The depolarization caused by Ba2+ was explained by a block of K+ channels.

    (4) Metabolic inhibition with protonophores (DNP and FCCP) is associated with specific effects on the plasmalemma in rat astrocytes in addition to the inhibition of the cellular ATP production. These agents should therefore be used with caution in studies of energy dependent mechanisms at the cellular level.

    (6) Neuroblastoma SK-N-SH cells and esthesioneuroblastoma Tp-410 cells have prominent Na+ currents. Both activation .and inactivation of the Na+ channels differed in their time and voltage dependence between these two types of neuroblastoma ce11s.

    Aim: To describe the prevalence of early complications in an unselected population of patients with type 1 diabetes mellitus (DM1) diagnosed in childhood with intensive insulin treatment from diagnosis.

    Methods: Eighty children and adolescents with DM1, age 7-22 years and DM1 duration >3 years, were studied. Neuropathy was defined as abnormal nerve conduction finding in ≥2 of 4 nerves (sural and peroneal nerves), nephropathy as albumin excretion rate ≥20 μg/min and retinopathy as all grades of retinal changes in fundus photographs.

    Results: The prevalence of neuropathy was 59%, of retinopathy 27% and of nephropathy 5% after 13 years DM1 duration. Mean (SD) long-term HbA1c was 8.4 (0.9)% (DCCT-corrected value).

    Conclusion: Even in a population with intensive insulin treatment from the beginning and fairly good metabolic control, the prevalence of subclinical neuropathy was high, while other diabetic complications were lower than usually reported.

    This article reviews studies of upper airway muscles in humans, with emphasis on muscle fiber structural and electrophysiological changes observed in patients with obstructive sleep apnea syndrome (OSAS). The concept of OSAS as a progressive disease is discussed and also possible causes. These include local nervous lesions in the upper airway, both motor and sensory. Previous muscle biopsy studies have given evidence for motor neuron lesions such as, e.g., the phenomenon of type grouping in histological sections. New data obtained with concentric needle EMG recordings from the palatopharyngeus muscles are also presented. In 10/12 OSAS patients there were typical findings indicating motor neuropathy (reduced EMG activity at maximal voluntary effort, long and polyphasic motor-unit potentials and, in two cases, spontaneous denervation activity), whereas such findings were only present in 3/15 patients with habitual snoring. This supports the hypothesis that progression from habitual snoring to the clinical disease of OSAS could be attributed to peripheral neurogenic lesions. © 2005 Elsevier B.V. All rights reserved.

    In the face of the rapid developments in sleep medicine, this book seeks to present the current knowledge in the pathophysiology, clinical presentation, diagnosis, and treatment of sleep apnea. New physiological approaches to modeling sleep and recent pathophysiologic findings in upper airway mechanics as well as the importance of inflammatory and oxidative processes and the underlying genetic aspects are discussed to open up new avenues of investigation for better understanding and improved therapeutic options. Besides the well-known CPAP therapy, chapters describe novel therapeutic methods that are currently under investigation and highlight their future prospects, limitations as well as recommendations for practice. The influence of breathing disturbances on the cardiovascular system during sleep is significant. Reviews of central sleep apnea and of cardiovascular complications of sleep-disordered breathing therefore play an important role in this edition. Some of the chapters are dedicated to specific aspects in children, in the elderly and in pregnancy and receive special attention as sleep apnea is relevant to all ages.     Abstract [en]

    This study, comprising six separate papers, is concerned with the  description and evaluation of some of the motor disturbances seen in Parkinson patients. Motor performances with different complexity in movement were investigated in patients with ideopathic Parkinson's disease. Muscle contraction characteristics were studied by electrical stimulation of the ulnar nerve and recording of the tension in the adductor pollicis muscle. Voluntary muscle contraction over a "single" joint was tested by concentric and eccentric isokinetic strength measurement including a stretch-shortening cycle. A semiautomatic voluntary movement with important postural components was tested by assessment of basal gait characteristics. Clinical scoring was performed according to Webster and the Hoehn and Y ahr rating scale. The relevance of the tests for evaluation of Parkinson's disease was tested by, a) withdrawal of medication, b) physiotherapeutic training, c) comparison with control subjects of the same age as the patients.The results showed that the contraction and relaxation characteristics of muscle contraction were shorter than in normals, but was not influenced by withdrawal of medication. Isokinetic muscle strength in ankle dorsiflexors in patients were lower in all contractions types andangular velocities, when compared to controls. Strength impainnent was greater in men than women. Strength was influenced by withdrawal of medication. Muscle contraction was performed with a lower efficiency than in nonnals. Muscle stiffness did not seem to impair strength. Gait in patients was disturbed with a shorter stride length and a lower gait velocity than in normals.Gait parameters were further disturbed, when medication was withdrawn. Nonnalization to constant velocity and constant stride duration improved the analyzis. Men were found to be more disabled than women. Gait, but not strength, correlated to the clinical score in group comparisons, but in intraindividual comparisons a correlation between strength and clinical score was found. Gait and strength parameters were not influenced by physical group training.

    The conclusion of the study was that motor analysis, employing different levels of complexity in motion, can be useful in the description of motor impairment and therapy in Parkinson's disease.
