    Abstract [en]

    Objective: Patients with eating disorders (ED) often suffer from co-morbid depression, which may complicate the ED treatment. Previous studies have found that ED interventions seem to have limited capacity to reduce depressive symptoms. Several studies of interpersonal psychotherapy (IPT), have found that when patients have been treated for depression, co-morbid symptoms have diminished. As depression and EDs are commonly co-occurring conditions, this pilot study aimed to examine the effect of an IPT treatment for these conditions, with the focus on the depressive symptoms. Method: In this multi-centre study, 16 patients with EDs and co-occurring major depression received 16 weeks of depression-focused IPT. Results: Significant improvements with substantial effect sizes were found for both depression (d = 1.48) and ED (d =.93). Symptom reduction in the two syndromes were strongly correlated (r =.625, p =.004). Patients with a restrictive ED did not improve on either depression or ED symptoms. Conclusion: These findings point to the usefulness of IPT for concurrent depression and ED with a bingeing/purging symptomatology. Working with negative affect and problem-solving related to current interpersonal problems may alleviate general psychological distress among these patients. © 2017 Informa UK Limited, trading as Taylor amp; Francis Group.

    Abstract [en]

    The purpose of this study has been to research how to implement a graphical interface for presenting information security information to management. The major conclusion of the study is that management use this kind of information mainly for financial and strategic matters. Hence the information must be presented in a way that enhances this use of the information.

    The study also concludes that people act insecure mainly due to:

    a) Insufficient knowledge of how/why to act secure.

    b) The users do not want to act secure due to social and organisational factors.

    To fight the first factor, the management need a tool that helps them to see where to spend their resources. To fight the second factor, the organisation needs to be well educated and the company culture should allow the users to act secure.

    Three heuristics for the design of information security solutions for management and a design solution for the interface are also presented in the study. The three heuristics are:

    1. Provide overview information very early in the program. The ordinary manager does not have the time or the knowledge to make this overview by himself/herself.

    2. Do not overwhelm the user. The ordinary management man/woman is not interested in the details of the information security and/or do not have time to read this sort of information. If he or she wants to access the details, he or she is likely to find them (if they are placed in a logical place).

    3. Provide information in a way that is common to the manager. Use wordings that the user understands. Provide contextual help for expressions that must be presented in a technical way.

    Fear avoidance och acceptans som mediatorer vid tinnitusbesvär: Två modeller jämförs i en enkätstudie2012Självständigt arbete på avancerad nivå (masterexamen), 20 poäng / 30 hpStudentuppsats (Examensarbete)

    Syftet med denna enkätstudie var att jämföra två olika förklaringsmodeller för tinnitusbesvär; fear avoidance och acceptans. Förutom att undersöka sambanden till tinnitusbesvär, testades även modellerna mot utfallsmåtten tinnitusvigilans och livskvalitet. Då nivå av depression och ångest är kända prediktorer för tinnitusbesvär, användes de som kontrollvariabler. Även processerna mindfulness och värderad riktning testades. Deltagare i studien var 362 patienter med tinnitus som någon gång mellan åren 2004-2011 varit i kontakt med Öronkliniken Hörselvården vid universitetssjukhuset i Linköping. Hierarkiska regressionsanalyser visade att både acceptans och fear avoidance förklarade unika bidrag till utfallsmåtten tinnitusbesvär och tinnitusvigilans. Värderad riktning förklarade unik varians i utfallsmåttet livskvalitet. Utifrån multipla mediationsanalyser visade det sig att både acceptans och fear avoidance medierade sambandet mellan ljudnivån av tinnitus och tinnitusbesvär samt sambandet mellan ljudnivån av tinnitus och tinnitusvigilans. Ingen av mediatorerna bidrog med signifikant mer förklaringsvärde än den andra. Slutsatsen är att acceptans och fear avoidance är två viktiga och kompletterande modeller för att förklara graden av tinnitusbesvär och tinnitusvigilans. Resultatet från denna studie kan ha implikationer för utvecklingen av mer effektiva behandlingar för tinnitus.

    Syftet med studien är att beskriva hur kvinnor som fått diagnosen borderline personliglighetsstörning upplever att deras liv har varit innan, under och efter diagnostiseringen. Syftet är också att ge en inblick i hur dessa kvinnor beskriver begreppet borderline. Studien har en kvalitativ ansats med narrativ teori som utgångspunkt, där sex kvinnor med diagnosen BPD har intervjuats. En inblick i informanternas subjektiva upplevelser har strävats efter genom berättelser. Detta för en förståelse för hur de ser på sin omvärld och sig själva. Resultatet visade en tendens bland informanterna att fly, vilket ofta grundats i en otillräcklighetskänsla. Tillståndet har främst präglats av impulsivitet och ett skiftande mellan ytterligheter. Att diagnostiseras med BPD har medfört en känsla av lättnad, men har också orsakat att informanterna blivit stigmatiserade. Resultatet påvisade även ett samband mellan ålder och valet av att tala öppet om diagnosen.

    The purpose of this study was to analyze the extent to which a sample of Swedish psychotherapists used techniques and interventions that are not part of the methods that they have been trained in. i.e. to what extent do they have an integrated approach in their professional activities. The hypothesis is that most psychotherapists in Sweden do not work strictly with just one psychotherapeutic orientation, but that they use techniques and methods from other orientations, which would be consistent with international research results in this field. Participants were recruited by mailed letters to a sample of authorized Swedish psychotherapists of all potential orientations. The letters contained an information paper and a questionnaire.

    The study of signed languages allows the dissociation of sensorimotor and cognitive neural components of the language signal. Here we investigated the neurocognitive processes underlying the monitoring of two phonological parameters of sign languages: handshape and location. Our goal was to determine if brain regions processing sensorimotor characteristics of different phonological parameters of sign languages were also involved in phonological processing, with their activity being modulated by the linguistic content of manual actions. We conducted an fMRI experiment using manual actions varying in phonological structure and semantics: (1) signs of a familiar sign language (British Sign Language), (2) signs of an unfamiliar sign language (Swedish Sign Language), and (3) invented nonsigns that violate the phonological rules of British Sign Language and Swedish Sign Language or consist of nonoccurring combinations of phonological parameters. Three groups of participants were tested: deaf native signers, deaf nonsigners, and hearing nonsigners. Results show that the linguistic processing of different phonological parameters of sign language is independent of the sensorimotor characteristics of the language signal. Handshape and location were processed by different perceptual and task-related brain networks but recruited the same language areas. The semantic content of the stimuli did not influence this process, but phonological structure did, with nonsigns being associated with longer RTs and stronger activations in an action observation network in all participants and in the supramarginal gyrus exclusively in deaf signers. These results suggest higher processing demands for stimuli that contravene the phonological rules of a signed language, independently of previous knowledge of signed languages. We suggest that the phonological characteristics of a language may arise as a consequence of more efficient neural processing for its perception and production.

    Early deafness results in crossmodal reorganization of the superior temporal cortex (STC). Here, we investigated the effect of deafness on cognitive processing. Specifically, we studied the reorganization, due to deafness and sign language (SL) knowledge, of linguistic and nonlinguistic visual working memory (WM). We conducted an fMRI experiment in groups that differed in their hearing status and SL knowledge: deaf native signers, and hearing native signers, hearing nonsigners. Participants performed a 2-back WM task and a control task. Stimuli were signs from British Sign Language (BSL) or moving nonsense objects in the form of point-light displays. We found characteristic WM activations in fronto-parietal regions in all groups. However, deaf participants also recruited bilateral posterior STC during the WM task, independently of the linguistic content of the stimuli, and showed less activation in fronto-parietal regions. Resting-state connectivity analysis showed increased connectivity between frontal regions and STC in deaf compared to hearing individuals. WM for signs did not elicit differential activations, suggesting that SL WM does not rely on modality-specific linguistic processing. These findings suggest that WM networks are reorganized due to early deafness, and that the organization of cognitive networks is shaped by the nature of the sensory inputs available during development.

    During the last two decades, Internet-delivered cognitive behavior therapy (ICBT) has been tested in hundreds of randomized controlled trials, often with promising results. However, the control groups were often waitlisted, care-as-usual or attention control. Hence, little is known about the relative efficacy of ICBT as compared to face-to-face cognitive behavior therapy (CBT). In the present systematic review and meta-analysis, which included 1418 participants, guided ICBT for psychiatric and somatic conditions were directly compared to face-to-face CBT within the same trial. Out of the 2078 articles screened, a total of 20 studies met all inclusion criteria. Results showed a pooled effect size at post-treatment of Hedges g = .05 (95% CI, -.09 to .20), indicating that ICBT and face-to-face treatment produced equivalent overall effects. Study quality did not affect outcomes. While the overall results indicate equivalence, there have been few studies of the individual psychiatric and somatic conditions so far, and for the majority, guided ICBT has not been compared against face-to-face treatment. Thus, more research, preferably with larger sample sizes, is needed to establish the general equivalence of the two treatment formats.

    The present study validated a Swedish version of the 47‐item Coaching Behavior Scale for Sport (CBS‐S). Sample 1 consisted of 506 team sport athletes [262 men and 244 women; mean age: 22.20, standard deviation (SD) = 3.90] distributed across 41 coaches at the two highest national levels of various sports. Athletes completed the CBS‐S and established questionnaires of coaching behaviors (LSS), self‐confidence (CSAI‐2R), and coach–athlete relationship (CART‐Q). An additional sample of 39 basketball players (21 men and 18 women; mean age = 17.40, SD = 2.39) completed the CBS‐S twice, approximately 4 weeks apart. Confirmatory factor analysis showed an acceptable model fit for the seven‐factor version of the CBS‐S, although two items of the negative personal rapport subscale displayed insufficient factor loadings. Correlations between the subscales of the CBS‐S and established instruments were in accordance with theoretical expectations, supporting the concurrent validity. Cronbach's alpha (> 0.82) for all dimensions provided support for the reliability of the CBS‐S, and test‐retest correlations indicated moderate stability over time. Cultural differences in the assessment of coaching behaviors and the usability of the CBS‐S by coaches for self‐reflection and development are discussed.

    Syftet med denna examensuppsats var att mäta kunskaper som deltagarna i en internetbehandlingen för generaliserat ångestsyndrom fått av psykoedukationen i behandlingen. I studien undersöktes om behandlingsgruppen ett år efter avslutad behandling kom ihåg det som lärdes ut i behandlingen. Det undersöktes även om det fanns något samband mellan kunskap och grad av oro. För att mäta kunskap framställdes ett flervalstest som baserades på modulerna i internetbehandlingen. Det slutgiltiga testet bestod av 22 items. Flervalstestet gavs ut till behandlingsgruppen i internetbehandlingen och till en kontrollgrupp. Behandlingsgruppen fick högre poäng på testet än kontrollgruppen. Det fanns ett samband mellan poäng på testet och poäng på PSWQ. Resultaten visar att kunskap har viss betydelse för hur personer med GAD upplever sin oro. Enbart kunskap verkar dock inte vara tillräckligt för att uppnå förbättring utan individen tycks även behöva internalisera och praktisera kunskapen.

    Syftet med studien var att beskriva individers upplevelser kring barndomstrauma och hur återhämtningsprocessen ter sig. Detta undersöktes via en webbaserad enkät. Studien har en kvalitativ ansats med tematisk analys som analysmetod och webbaserad enkät som insamlingsmetod. Sammanlagt deltog 20 personer i studien, 19 kvinnor och 1 man. Resultatet visade att informanterna rapporterade en upplevd svårighet att hantera traumat under barndomen då misshandeln pågick samt en upplevelse av bristande stöd från den vuxna omgivningen. Många av informanterna ansåg att detta kan ha varit anledningen till att de hamnade i destruktiva relationer i vuxen ålder. Resultatet visade även på att informanterna beskrev olika återhämtningsprocesser och baserat på deras beskrivningar gick det inte att antyda en återhämtningsprocess förrän de tagit sig ur destruktiva relationer. Informanterna beskrev att barndomstraumat har följt med dem hela livet och återhämtningen var en ständigt pågående process.

    Williams (WS) and Down (DS) syndromes have been associated with specifically compromised short-term memory (STM) subsystems. Individuals with WS have shown impairments in visuospatial STM, while individuals with DS have often shown problems with the recall of verbal material. However, studies have not usually compared the development of STM skills in these domains, in these populations. The present study employed a cross-sectional developmental trajectories approach, plotting verbal and visuospatial STM performance against more general cognitive and chronological development, to investigate how the domain-specific skills of individuals with WS and DS may change as development progresses, as well as whether the difference between STM skill domains increases, in either group, as development progresses. Typically developing children, of broadly similar cognitive ability to the clinical groups, were also included. Planned between- and within-group comparisons were carried out. Individuals with WS and DS both showed the domain-specific STM weaknesses in overall performance that were expected based on the respective cognitive profiles. However, skills in both groups developed, according to general cognitive development, at similar rates to those of the TD group. In addition, no significant developmental divergence between STM domains was observed in either clinical group according to mental age or chronological age, although the general pattern of findings indicated that the influence of the latter variable across STM domains, particularly in WS, might merit further investigation.

    Studien hade till syfte att adaptera och validera den multidimensionella stressenkäten Dundee Stress State Questionnaire (DSSQ) som mäter stress utifrån 12 skalor som sammanfaller kring tre högre dimensioner: uppgiftsfokus, ångest och oro. DSSQ adapteras från originalspråket, engelska, till svenska. Tillämpbarheten för den svenska versionen av DSSQ undersöks genom att studera enkätens reliabilitet och validitet samt genom att tillämpa enkäten i två olika kontext, där två olika typer av uppgifter genomförs, ett arbetsminnestest och en polisstudentövning.

    Sammantaget visade resultatet att den svenska adaptionen av DSSQ har en hög reliabilitet. Dock gick det inte att bekräfta att den svenska adaptionen av DSSQ bibehåller samma faktormodell som originalversionen av DSSQ. Den statistiska analysen visade att de förändringar som identifierats i deltagarnas sinnesstämning delvis stämmer överens med tidigare forskning. Slutsatsen är att fler deltagare behövs för att validera faktormodellen hos den svenska adaptionen av DSSQ och den svenska adaptionen av DSSQ anses vara tillämpar i forskning då den visar på god reliabilitet.

    Studies have shown that many problems office workers face today arise from digital communication and collaboration, with information overload being the biggest obstacle to being effective and productive. Briteback is a new messaging app for teams, which was launched in November 2015, and which aims to solve these problems by providing users with a platform where they can manage all their work-related communication, in an attempt to structure office workers’ communication and to relieve communication related stress. As the app is a new competitor on the market, Briteback is evaluating the possibility of using different marketing strategies to position itself against competition, and to deepen their customer’s engagement and loyalty. According to several studies, gamification is a promising marketing strategy which can be applied to all kind of contexts. Given this background, this study examines what elements a gamification concept could include to assist Briteback with its marketing strategy to acquire new customers, as well as to deepen customer engagement and loyalty. The gamification concept resulted in an achievement based model, where users of the app can collect badges and learn more about the app and its features, as well as how to use these most effectively. Users can compete against each other for a leaderboard position and earn a VIP club membership to gain early access to new features. Usability tests at the end of the design process showed that study participants rated the overall usability of the gamification concept with a score of 75.6. It is concluded that the developed gamification elements have great potential of assisting Briteback with its marketing goals.

    Visual capacity generally declines as people age, yet its impact on the visual search patterns along sections of different road during actual driving still remains undocumented. This onroad driving study simultaneously recorded 30 older drivers eye movement and precise vehicle movement trajectories. The vehicle positions were linked to every identified eye fixation for each individual driver, so that the locations of the drivers gaze origin in geospatial coordinates were obtained. Spatial distribution pattern of drivers eye fixations were then mapped and analysed. In addition, the associations between older drivers visual capacity (processing speed, divided and selective attention) and their eye fixation patterns in various driving manoeuvres were investigated. The results indicate that driving scenarios have a significant impact on older drivers visual patterns. Older drivers performed more frequent eye fixations at roundabouts, while they tended to fixate on certain objects for longer periods during straight road driving. The key findings show that the processing speed and divided attention of older drivers were associated with their eye fixations at complex right-turns; drivers with a lower capacity in selective attention performed less frequent eye fixations at roundabouts. This study has also demonstrated that visualisation and spatial statistics are effective and intuitive approaches to eye movement analysis.

    The symptomatology of autism spectrum disorder (ASD) can make driving risky, but little is known about the on-road driving behaviour of individuals with ASD. This study assessed and compared the on-road driving performance of drivers with and without ASD, and explored how the symptomatology of ASD hinders or facilitates on-road driving performance. Sixteen drivers with ASD and 21 typically-developed drivers participated in the study. Drivers with ASD underperformed in vehicle manoeuvring, especially at left-turns, right-turns and pedestrian crossings. However, drivers with ASD outperformed the TD group in aspects related to rule-following such as using the indicator at roundabouts and checking for cross-traffic when approaching intersections. Drivers with ASD in the current study presented with a range of capabilities and weaknesses during driving.

    Syftet var att undersöka samband mellan stress, hälsoångest, sjukvårdsbesök samt läkemedelsförbrukning i fängelsemiljö. Respondenterna var 66 män i åldrarna 21 - 68 år med avtjänade straff från en månad till tretton år och nio månader. Studien baserades på självskattningsfrågeformulär vilket mäter stress  (Perceived Stress Scale, PSS ) och hälsoångest (Health Anxiety Inventory, Short Version, SHAI). Dataanalyser kontrollerade samband, predicerbarhet och interaktionseffekter. Resultatet påvisade samband och predicerbarhet mellan stress och hälsoångest samt med hur ofta intagna besöker sjukmottagningen men inget samband med förbrukning av läkemedel. Samband påvisades mellan stress och hög hälsoångest. Coping påvisade samband med låg hälsoångest medan ökade sjukvårdsbesök påvisades ha samband med hög hälsoångest. Slutsatser i studien är att stress fungerar som en möjlig påverkande faktor för såväl hälsoångest som sjukvårdbesök. Andra slutsatser är att när få yttre möjligheter till coping finns riktas fokus mot inre skeenden och kroppssensationer vilket utvecklar ytterligare riskfaktorer för att utveckla stress och hälsoångest.

    Evaluation of the attachment scale in the Trauma Symptom Inventory-2: Parental experiences of traumatic events and close relationships2013Självständigt arbete på avancerad nivå (masterexamen), 10 poäng / 15 hpStudentuppsats (Examensarbete)
    Abstract [en]

    The aim of this study was to evaluate the psychometric properties of the attachment scale added in the newly developed self-rating questionnaire Trauma Symptom Inventory-2 (TSI-2). Participants were recruited from the Swedish parent-infant unit Hagadal (N=58). Reliability analyses concluded Cronbach´s α .92 for attachment total scale, .88 for avoidance subscale, and .91 for rejection sensitivity subscale. Convergent validity analyses concluded moderate to strong correlations between TSI-2 attachment scale and subscales, and Experiences in Close Relationships (ECR) total scale and subscales (r= .34 - .68, p ≤ .01). Criterion validity analyses concluded that adverse childhood circumstances measured by Linköping Youth Life Experiences Scale (LYLES) signficantly estimated 17 % of variance in TSI-2 attachment scale scores. Preliminary support for reliability and validity of the TSI-2 attachment scale was obtained. No previous trauma symptom rating instrument has included information about adult attachment styles. The present findings point to the benefits of such inclusion.

    The present study explored the environmental and genetic etiologies of the longitudinal relations between prereading skills and reading and spelling. Twin pairs (n = 489) were assessed before kindergarten (M = 4.9 years), post-first grade (M = 7.4 years), and post-fourth grade (M = 10.4 years). Genetic influences on five prereading skills (print knowledge, rapid naming, phonological awareness, vocabulary, and verbal memory) were primarily responsible for relations with word reading and spelling. However, relations with post-fourth-grade reading comprehension were due to both genetic and shared environmental influences. Genetic and shared environmental influences that were common among the prereading variables covaried with reading and spelling, as did genetic influences unique to verbal memory (only post-fourth-grade comprehension), print knowledge, and rapid naming.

    Kvinnors upplevelser av vården efter att ha drabbats av förlossningsskador har fått stor uppmärksamhet i media det senaste året. Den nationella forskningen kring drabbade kvinnors upplevelser av vårdpersonalens bemötande är dock knapphändig. Syftet med studien var att utforska kvinnors upplevelser av bemötandet inom vården efter att ha drabbats av en förlossningsskada och eventuella psykologiska konsekvenser till följd av bemötandet. Data inhämtades genom individuella semi-strukturerade intervjuer med 14 kvinnor som drabbats av en förlossningsskada. De transkriberade intervjuerna bearbetades därefter med tematisk analys. Vi identifierade fem teman; Vården är inte anpassad för oss, Jättebra att de fokuserar på barnet - men jag då?, Det gör ont att föda barn, Jag antar att det var mitt fel då och slutligen Jag utsätter mig inte för detta igen. Sammanfattningsvis kan konstateras att bemötandet är en viktig faktor för dessa kvinnors välmående och syn på en eventuell nästa förlossning.

    BACKGROUND: It is not clear whether cognitive behavior therapy (CBT) works through changing dysfunctional thinking. Although several primary studies have examined the effects of CBT on dysfunctional thinking, no meta-analysis has yet been conducted.

    METHOD: We searched for randomized trials comparing CBT for adult depression with control groups or with other therapies and reporting outcomes on dysfunctional thinking. We calculated effect sizes for CBT versus control groups, and separately for CBT versus other psychotherapies and respectively, pharmacotherapy.

    RESULTS: 26 studies totalizing 2002 patients met inclusion criteria. The quality of the studies was less than optimal. We found a moderate effect of CBT compared to control groups on dysfunctional thinking at post-test (g=0.50; 95% CI: 0.38-0.62), with no differences between the measures used. This result was maintained at follow-up (g=0.46; 95% CI: 0.15-0.78). There was a strong association between the effects on dysfunctional thinking and those on depression. We found no significant differences between CBT and other psychotherapies (g=0.17; p=0.31), except when restrict in outcomes to the Dysfunctional Attitudes Scale (g=0.29). There also was no difference between CBT and pharmacotherapy (g=0.04), though this result was based on only 4 studies.

    DISCUSSION: While CBT had a robust and stable effect on dysfunctional thoughts, this was not significantly different from what other psychotherapies or pharmacotherapy achieved. This result can be interpreted as confirming the primacy of cognitive change in symptom change, irrespective of how it is attained, as well as supporting the idea that dysfunctional thoughts are simply another symptom that changes subsequent to treatment.

    Over the past few decades new ways of conceiving the relation between people, practices and institutions have been developed, enabling an understanding of human conduct in complex situations that is distinctive from traditional psychological and sociological conceptions. This distinctiveness is derived from a sophisticated analytic approach to social action which combines conversation analysis with the fresh treatment of epistemology, mind, cognition and personality developed in discursive psychology. This text is the first to showcase and promote this new method of discursive research in practice.     Touch can be highly emotional, and depending on the environment, it can be perceived as pleasant and comforting or disgusting and dangerous. Here, we studied the impact of context on the processing of tactile stimuli using a functional magnetic resonance imaging (fMRI) paradigm. This was achieved by embedding tactile stimulation in a variable olfactory environment. Twenty people were scanned with BOLD fMRI while receiving the following stimulus blocks: Slow stroking Touch, Civette odor (feces like), Rose odor, Touch + Civette, and Touch + Rose. Ratings of pleasantness and intensity of tactile stimuli and ratings of disgust and intensity of olfactory stimuli were collected. The impact of the olfactory context on the processing of touch was studied using covariance analyses. Coupling between olfactory processing and somatosensory processing areas was assessed with psychophysiological interaction analysis (PPI). A subjectively disgusting olfactory environment significantly reduced the perceived pleasantness of touch. The touch fMRI activation in the secondary somatosensory cortex, operculum 1 (OP1), was positively correlated with the disgust towards the odors. Decreased pleasantness of touch was related to decreased posterior insula activity. PPI analysis revealed a significant interaction between the OP1, posterior insula, and regions processing the disgust of odors (orbitofrontal cortex and amygdala). We conclude that the disgust evaluation of the olfactory environment moderates neural reactivity in somatosensory regions by upregulation of the OP1 and downregulation of the posterior insula. This adaptive regulation of affective touch processing may facilitate adaptive reaction to a potentially harmful stimulus. (C) 2016 Elsevier Inc. All rights reserved.

    BACKGROUND: Although treatment guidelines suggest that suicidal patients with depression should be treated for depression with psychotherapy, it is not clear whether these psychological treatments actually reduce suicidal ideation or suicide risk.

    METHODS: We conducted a systematic review and meta-analysis of studies on psychotherapy for depression in which outcomes on suicidality were reported. We also focused on outcomes on hopelessness because this is strongly associated with suicidal behavior in depression.

    RESULTS: Thirteen studies (with 616 patients) were included, three of which examined the effects of psychotherapy for depression on suicidal ideation and suicide risk, and eleven on hopelessness. No studies were found with suicide attempts or completed suicides as the outcome variables. The effects on suicidal ideation and suicide risk were small (g=0.12; 95% CI: -0.20-0.44) and not statistically significant. A power calculation showed that these studies only had sufficient power to find an effect size of g=0.47. The effects on hopelessness were large (g=1.10; 95% CI: 0.72-1.48) and significant, although heterogeneity was very high. Furthermore, significant publication bias was found. After adjustment of publication bias the effect size was reduced to g=0.60.

    DISCUSSION: At this point, there is insufficient evidence for the assumption that suicidality in depressed patients can be reduced with psychotherapy for depression. Although psychotherapy of depression may have small positive effects on suicidality, available data suggest that psychotherapy for depression cannot be considered to be a sufficient treatment. The effects on hopelessness are probably higher.

    Abstract [en]

    The effects of non-directive supportive therapy (NDST) for adult depression have been examined in a considerable number of studies, but no meta-analysis of these studies has been conducted. We selected 31 studies on NDST from a comprehensive database of trials, examining psychotherapies for adult depression, and conducted meta-analyses in which NDST was compared with control groups, other psychotherapies and pharmacotherapy. We found that NDST is effective in the treatment of depression in adults (g=0.58; 95% CI: 0.45-0.72). NDST was less effective than other psychological treatments (differential effect size g=-0.20; 95% CI: -0.32 to -0.08, p<0.01), but these differences were no longer present after controlling for researcher allegiance. We estimated that extra-therapeutic factors (those processes operating in waiting-list and care-as-usual controls) were responsible for 33.3% of the overall improvement, non-specific factors (the effects of NDST compared with control groups) for 49.6%, and specific factors (the effects of NDST compared with other therapies) for 17.1%. NDST has a considerable effect on symptoms of depression. Most of the effect of therapy for adult depression is realized by non-specific factors, and our results suggest that the contribution of specific effects is limited at best.

    Abstract [en]

    Personalized medicine is aimed at identifying which characteristics of an individual predict the outcome of a specific treatment, in order to get a better match between the individual and the treatment received. We conducted a systematic review and meta-analysis of randomized trials comparing two psychotherapies directly in a group of depressed patients with a specific characteristic. We focused on the six most examined types of psychotherapy for adult depression. Our searches resulted in 41 studies with 2,741 patients who met inclusion criteria. These 41 studies examined 27 specific characteristics of patients. Power calculations indicated that we would need 4 studies for each characteristic to find a clinically relevant effect size set at g = 0.50 and 16 studies for an effect size of 0.24. Only 3 patient characteristics were found to have sufficient power and to significantly moderate treatment outcomes. Cognitive-behavioral therapy was found to be more effective than other therapies in older adults (g= 0.29), in patients with comorbid addictive disorders (g = 0.31), and in university students (g = 0.46). Risk of bias was considerable in most of the included studies. It was estimated that it will take another 326 years to have sufficient statistical power for showing an effect size of g = 0.50 of the 27 characteristics, and 1,372 years to show an effect size of 0.24. Although several dozens of studies have compared the effects of psychotherapies in specific target groups, we will need to develop more powerful alternatives to comparative outcome studies in order to identify personalized treatments for depression.

We conducted a systematic review and meta-analysis of randomized trials comparing two treatments directly in a group of patients with a specific characteristic.

    METHODS: We searched relevant studies from bibliographical databases and included trials comparing (1) medication with psychotherapy, (2) medication with combined treatment, and (3) psychotherapy with combined treatment, in specific target groups (a) with a predefined sociodemographic characteristic, (b) a specific type of depression, (c) a comorbid mental or somatic disorder, or (d) from a specific setting (outpatients, primary care).

    RESULTS: We included 52 studies with 4,734 depressed patients. In these studies, 20 characteristics of the target groups were examined. The results showed that medication is probably the best treatment for dysthymia, and combined treatments are more effective in depressed outpatients, as well as in depressed older adults. However, in order to examine the 20 characteristics in the three categories of comparisons, 254 studies would be needed for having sufficient statistical power to show an effect size of g = 0.5. Currently, only 20.1% of these studies have been conducted.

    CONCLUSIONS: Although a considerable number of studies have compared medication, psychotherapy, and combined treatments, and some preliminary results are useful for deciding which treatment is best for which patient, the development of personalized treatment of depression has only just begun.

    Recent years have seen a near-doubling of the number of studies examining the effects of psychotherapies for generalized anxiety disorder (GAD) in adults. The present article integrates this new evidence with the older literature through a quantitative meta-analysis. A total of 41 studies (with 2132 patients meeting diagnostic criteria for GAD) were identified through systematic searches in bibliographical databases, and were included in the meta-analysis. Most studies examined the effects of cognitive behavior therapy (CBT). The majority of studies used waiting lists as control condition. The pooled effect of the 38 comparisons (from 28 studies) of psychotherapy versus a control group was large (g=0.84; 95% CI: 0.71-0.97) with low to moderate heterogeneity. The effects based on self-report measures were somewhat lower than those based on clinician-rated instruments. The effects on depression were also large (g=0.71; 95% CI: 0.59-0.82). There were some indications for publication bias. The number of studies comparing CBT with other psychotherapies (e.g., applied relaxation) or pharmacotherapy was too small to draw conclusions about comparative effectiveness or the long-term effects. There were some indications that CBT was also effective at follow-up and that CBT was more effective than applied relaxation in the longer term.

    We conducted a meta-analysis of randomized trials in which the effects of treatment with antidepressant medication were compared to the effects of combined pharmacotherapy and psychotherapy in adults with a diagnosed depressive or anxiety disorder. A total of 52 studies (with 3,623 patients) met inclusion criteria, 32 on depressive disorders and 21 on anxiety disorders (one on both depressive and anxiety disorders). The overall difference between pharmacotherapy and combined treatment was Hedges' g = 0.43 (95% CI: 0.31-0.56), indicating a moderately large effect and clinically meaningful difference in favor of combined treatment, which corresponds to a number needed to treat (NNT) of 4.20. There was sufficient evidence that combined treatment is superior for major depression, panic disorder, and obsessive-compulsive disorder (OCD). The effects of combined treatment compared with placebo only were about twice as large as those of pharmacotherapy compared with placebo only, underscoring the clinical advantage of combined treatment. The results also suggest that the effects of pharmacotherapy and those of psychotherapy are largely independent from each other, with both contributing about equally to the effects of combined treatment. We conclude that combined treatment appears to be more effective than treatment with antidepressant medication alone in major depression, panic disorder, and OCD. These effects remain strong and significant up to two years after treatment. Monotherapy with psychotropic medication may not constitute optimal care for common mental disorders.

    Successful treatment of parental depression may have a positive effect on the functioning and psychopathology of their children. We conducted a meta-analysis to examine the effects of psychotherapy for depressed mothers on their children and parental functioning. We used a database of randomized controlled trials examining the effects of psychotherapy for adult depression and selected trials comparing psychotherapy and control conditions in depressed mothers and reporting outcomes in their children and parental functioning. Nine studies were included. The quality of these studies was not optimal and the outcome instruments differed considerably from each other. The therapies resulted in significantly decreased levels of depression (g = 0.66) in the mothers. In the seven studies that reported outcomes on the mental health of children, a significant effect size was also found (g = 0.40). The eight studies examining mother-child interactions resulted in a significant effect size of g = 0.35, and the five studies examining parenting/marital distress had a pooled effect size of g = 0.67. We found that psychotherapy leads to decreased levels of depression in depressed mothers and also found indications that psychotherapy may have a positive effect on the mental health of their children and parenting/marital distress. However, more high-quality research is needed before a definite answer can be given.

    Patienters upplevelser av internetbaserad självhjälpsbehandling är ett obeforskat område inom psykoterapiforskningen. I syfte att utforska upplevelser av att genomgå internetadministrerad kognitiv beteendeterapeutisk självhjälpsbehandling mot depression intervjuades 12 deltagare. Tematisk analys genererade de övergripande kategorierna:

    Läsa och göra; Någon som.. bryr sig; Det svåra är det givande samt Orden man bär med sig. Mönster av upplevelser och förändringsprocesser i informanternas berättelser föll ut i tre grupperingar: Noviser, Lärjungar och Mästare. Dessa mönster av upplevelser kan förstås utifrån tidigare studier av förändringsprocesser i psykoterapi. Kvalitativa studier på autonomi, kontext och funktionen av terapeutkontakt behövs för att ytterligare utreda förändringsprocesser i internetbaserad självhjälpsbehandling.

    Syftet med denna kvalitativa studie var att utforska hur psykoterapeuter upplever, påverkas av och hanterar samspelssituationer med patienter som har varit med om svåra livshändelser och som dissocierar under en specifik session. Åtta kvinnliga psykoterapeuter ingick i studien, samtliga yrkesverksamma inom specialiserad traumavård eller psykiatri.

    I studien har Interpretative Phenomenological Analysis (IPA) möjliggjort att mer i detalj ta del av den enskildes upplevelser men även utrymme för att analysera och tolka utifrån ett vidare teoretiserande perspektiv. Analysen och tolkningen av materialet resulterade i tre huvudteman: Upplevelsen av dissociation - relationsbrott i det terapeutiska rummet, Viljan till kontakt - den aktiva relationsupprättande terapeuten samt Relationen som grund - den trygghetsskapande terapeuten.

    Resultatet visade på att patienternas psykopatologi men även tidpunkten i behandlingen som terapeuterna hänvisade till hade stor betydelse för hur samspelsmönster utvecklades och vilka interventioner som kom i fokus. De dissociativa relationsbrotten eller identitetsväxlingarna uppmärksammades ofta på en ordlös, kroppslig nivå. Alliansarbetet och den tillitsskapande processen var en central uppgift som mycket av det övriga arbetet vilade på.

    Objective

    Internet-delivered cognitive behavior therapy (ICBT) has been developed and tested for treating persons with generalized anxiety disorder (GAD). A new form of CBT focuses on acceptance (of internal experiences or difficult psychological content), mindfulness and valued actions. To date this form of CBT has not been delivered via the internet for persons with GAD. The aim of this study was to describe the functionality of a new internet-delivered acceptance-based behavior therapy for GAD, and to test the effect of the intervention in an open pilot trial.

    Methods

    Following exclusion of two patients we included 14 patients diagnosed with GAD from two primary care clinics. At 2–3 months follow-up after treatment 10 patients completed the outcome measures. The treatment lasted for an average of 15 weeks and consisted of acceptance-based techniques, behavior therapy components and homework assignments.

    Results

    A majority of participants completed all modules during the treatment. Findings on the Penn State Worry Questionnaire showed a within-group improvement of Cohen's d = 2.14 at posttreatment. At the follow-up results were maintained. Client satisfaction ratings were high.

    Conclusions

    We conclude that internet-delivered acceptance-based behavior therapy potentially can be a promising new treatment for GAD. A controlled trial of the program has already been completed.

    The article by Blumen, Rajaram, and Henkel (2013) raises some very interesting research topics. Using the aging population as the prime example, they also provide general recommendations for future research in the area of collaborative memory; ‘it's time to become more applied’, and we appreciate such a suggestion.

    The article spans many subfields and for obvious reasons, it is not possible to consider every potential issue in this field in one single article. In addition, there are several issues that could be either extended or added. We will in this commentary focus on issues we consider important for the understanding of the current literature, and we will add some from our own research.

    The purpose of the experiment on collaborative memory was to investigate if the collaborative inhibition is due to collaborating pair's disruption of each others' retrieval strategies (the retrieval strategy disruption hypothesis, RSD). The participants' (N=36) task was to recall a list of 60 words individually and collaboratively. Retrieval strategies were manipulated by presenting word lists organised either by categories or by country of origin and adoption of retrieval strategies were examined by the adjusted ratio of clustering score. Half of the dyads received word lists organised by the same strategy and half of the dyads received word lists organised by different strategies. The results revealed a main effect of collaboration, i.e., collaborative recalled items were significantly fewer than the sum of the non-redundant individually recalled items. Both conditions (same strategies vs different strategies) suffered to the same extent from collaboration, which did not support the RSD hypothesis. However, focusing on words recalled individually but not collaboratively, dyads with different strategies, as predicted by the RSD, forgot more items during collaboration than did dyads with the same strategy. Additional results suggest that collaborative forgetting is mainly manifested by forgetting of non-overlapping items (as measured by individual recalls).

    Abstract [en]

    Numerous studies have reported subliminal repetition and semantic priming in the visual modality. We transferred thisparadigm to the auditory modality. Prime awareness was manipulated by a reduction of sound intensity level.Uncategorized prime words (according to a post-test) were followed by semantically related, unrelated, or repeated targetwords (presented without intensity reduction) and participants performed a lexical decision task (LDT). Participants withslower reaction times in the LDT showed semantic priming (faster reaction times for semantically related compared tounrelated targets) and negative repetition priming (slower reaction times for repeated compared to semantically relatedtargets). This is the first report of semantic priming in the auditory modality without conscious categorization of the prime.

    This Handbook illustrates the importance of examining child mental health from a different perspective, one that assumes that psychiatric categories are made real in and through both written and spoken language.     Radikal prostatektomi, den vanligaste kurativa behandlingen vid prostatacancer i Sverige, orsakar sexuella komplikationer såsom erektil dysfunktion, ejakulationsförlust och ofta en förändrad orgasmkänsla. I dag ökar intresset för rehabilitering i cancervården och i det nationella vårdprogrammet för prostatacancer betonas sexologisk rehabilitering.

    Forskning inom detta område är främst kvantitativ med störst fokus på erektil dysfunktion. Även den kvalitativa forskning som finns har huvudsakligen kretsat kring erektil dysfunktion. Denna studies syfte var att undersöka hur patienter efter kirurgisk behandling vid prostatacancer upplever orgasmförändring och ejakulationsförlust.

    This study examined the development of phonological recoding in short-term memory (STM) span tasks among two clinical groups with contrasting STM and language profiles: those with Down syndrome (DS) and Williams syndrome (WS). Phonological recoding was assessed by comparing: (1) performance on phonologically similar and dissimilar items (phonological similarity effects, PSE); and (2) items with short and long names (word length effects, WLE). Participant groups included children and adolescents with DS (n = 29), WS (n = 25) and typical development (n = 51), all with average mental ages around 6 years. The group with WS, contrary to predictions based on their relatively strong verbal STM and language abilities, showed no evidence for phonological recoding. Those in the group with DS, with weaker verbal STM and language abilities, showed positive evidence for phonological recoding (PSE), but to a lesser degree than the typical group (who showed PSE and WLE). These findings provide new information about the memory systems of these groups of children and adolescents, and suggest that STM processes involving phonological recoding do not fit with the usual expectations of the abilities of children and adolescents with WS and DS.     Modeling the effect of early age-related hearing loss on cognition and participation in social leisure activities2015Konferensbidrag (Övrigt vetenskapligt)
    Abstract [en]

    There are well-known age-related declines in hearing, cognition and social participation. Furthermore, previous studies have shown that hearing loss is associated with both cognitive decline and increased risk for social isolation and that engagement in social leisure activities is related to cognitive decline. However, it is unclear how the three concepts and age relate to each other. In the current study, behavioral measures of hearing and memory were examined in relation to self-reported participation in social leisure activities. Data from two different samples were analyzed with structural equation modeling. The first consisted of 297 adults from Umeå, Sweden, who participated in the Betula longitudinal study. The second consisted of 273 older adults who volunteered for lab-based research on aging in Toronto, Canada. Structural equation modeling yielded two models with similar statistical properties for both samples. The first model suggests that age contributes to both hearing and memory performance, hearing contributes to memory performance, and memory (but not hearing) contributes to participation in social leisure activities. The second model also suggests that age contributes to hearing and memory performance and that hearing contributes to memory performance, but that age also contributes to participation in social leisure activities, which in turn contributes to memory performance. The models were confirmed in both samples, indicating robustness in the findings, especially since the samples differed on background variables such as years of education and marital status. Few participants in both samples were candidates for hearing aids, but most of those who were candidates used them. This suggests that even early stages of hearing loss can increase demands on cognitive processing that may deter participation in social leisure activities.

    When speech is degraded, word report is higher for semantically coherent sentences (e.g., her new skirt was made of denim) than for anomalous sentences (e.g., her good slope was done in carrot). Such increased intelligibility is often described as resulting from “top–down” processes, reflecting an assumption that higher-level (semantic) neural processes support lower-level (perceptual) mechanisms. We used time-resolved sparse fMRI to test for top–down neural mechanisms, measuring activity while participants heard coherent and anomalous sentences presented in speech envelope/spectrum noise at varying signal-to-noise ratios (SNR). The timing of BOLD responses to more intelligible speech provides evidence of hierarchical organization, with earlier responses in peri-auditory regions of the posterior superior temporal gyrus than in more distant temporal and frontal regions. Despite Sentence content × SNR interactions in the superior temporal gyrus, prefrontal regions respond after auditory/perceptual regions. Although we cannot rule out top–down effects, this pattern is more compatible with a purely feedforward or bottom–up account, in which the results of lower-level perceptual processing are passed to inferior frontal regions. Behavioral and neural evidence that sentence content influences perception of degraded speech does not necessarily imply “top–down” neural processes.
