Abstract [sv]

Selektiv mutism (SM) är ett tillstånd där barn inte talar i vissa sociala situationer trots bevarad talförmåga i andra sammanhang. SM är klassificerat som ett ångestsyndrom i DSM-V och i ICD-11. Logopeder kan ha en roll i utredning och behandling av barn med SM, men nationella kliniska riktlinjer för logopediskt arbete med denna patientgrupp saknas i Sverige. Syftet medden föreliggande studien var att undersöka hur logopeder i Sverige arbetar med barn med SM. Mer specifikt adresserade frågeställningarna logopeders erfarenheter av hinder och möjligheter och deras syn på behovet av kliniska riktlinjer för arbetet med denna patientgrupp.

En kvalitativ design användes och materialet samlades in genom individuella semistrukturerade intervjuer med 14 legitimerade logopeder med erfarenhet från arbete medbarn med SM. Intervjuerna transkriberades ortografiskt och analyserades med reflexiv tematisk analys enligt Braun och Clarke (2022).

Analysen resulterade i fem teman: (1) professionell identitet och kompetensutveckling, (2) bedömning och diagnostik, (3) behandlingsmetoder, (4) teamarbete och samverkan samt (5) strukturella förutsättningar och behov av riktlinjer. Resultaten visade att SM ges begränsat utrymme i logopedprogrammens grundutbildning, vilket innebär att logopeder i hög grad bygger sin kompetens genom klinisk erfarenhet och egna initiativ. Bedömnings- och diagnosförfaranden varierade påtagligt mellan regioner, liksom val av behandlingsmetoder, med sliding in och shaping som de mest konsekvent använda metoderna. Samverkan med psykologer, pedagoger och föräldrar upplevdes som värdefull men var ofta informell eller saknades helt. Deltagarna uttryckte ett behov av nationella kliniska riktlinjer som adresserar utredningens innehåll, rekommenderade behandlingsmetoder, ansvarsfördelning mellan professioner samt hantering av komorbiditet.

Sammantaget visar studien att logopeder i Sverige ofta tar ansvar för en komplex patientgrupp under förhållanden präglade av strukturell ojämlikhet och avsaknad av nationellarekommendationer. Nationella kliniska riktlinjer och formaliserade tvärprofessionella team skulle kunna underlätta logopeders arbete och leda till att barnen får mer likvärdig och evidensbaserad vård.

Abstract [en]

Selective mutism (SM) is a condition in which children fail to speak in specific social situations despite having speech ability in others. SM is classified as an anxiety disorder in DSM-V and in ICD-11. Speech-language pathologists' (SLPs) often play a role in assessment and treatment of children with SM, but national clinical guidelines for speech and language pathologists'work with this patient group in Sweden are absent. The aim of the presentstudy was to examine how SLPs in Sweden work with children with SM. More specifically, research questions addressed SLPs experiences of barriers and facilitators, and their views on needs for clinical guidelines regarding work with this population.

A qualitative design was employed, using individual semi-structured interviews with 14 licensed SLPs with experience from working with children with SM. The interviews were transcribed orthographically and analysed using reflexive thematic analysis following Braun and Clarke (2022).

Five themes were identified: (1) professional identity and competence development, (2) assessment and diagnostics, (3) treatment methods,(4) interprofessional collaboration, and (5) structural conditions and the need for guidelines. The results showed that SM receives minimal coverage in SLP education, leaving clinicians to build competence through experience and self-directed learning. Assessment and diagnostic practices varied considerably across regions, as did treatment approaches, with sliding-in and shaping as the most consistently used methods. Collaboration with psychologists, teachers and parents was experienced as valuable but often informal or absent. Participants identified a need for national clinical guidelines addressing assessment procedures, treatment recommendations, role allocation between professions, and the management of comorbidities.

The findings suggest that SLPs in Sweden often assume responsibility for a complex patient group under conditions marked by structural inequality and a lack of national recommendations. National clinical guidelines and formalised interprofessional teams wouldfacilitate the work of SLPs and lead to children receiving more equal and evidence-based care.
